We are back to using silicone gel filled implants under the muscle . I have seen the full circle, including a period where we basically removed a lot of implants and not much was done as augmentation.
Both saline filled and silicone filled implants are safe and can be used, depending on the coverage over the implant. If the patient is very thin and has very little breast tissue, Silicone is preferred because one may feel some of the folds of the implants through the skin with saline implants. The pros and cons should be discussed with the patient and a final decision should be made.

The subpectoral breast augmentation is the preferred position since the complications are rare and the results extremely natural. I always believe that if you can tell a person has breast implants, that is not a natural result. The tendency to develop a "contracture" following a submammary (under breast tissue) augmentation is 20 times higher.
